Level editing types may want to check out RAWscape, a program that generates terrain from RAW files, which can be generated with most graphics programs (like Paintshop Pro) in an attempt to make a program that allows you to design a map simply by drawing it. The result is described as "not that simple," however, and therefore is only recommended for experienced map authors, but in that case it is said to "make it quite a bit easier and faster (and possible) to make complex terrains."
