There's a DOOM III Q&A on Gaming Invasion talking with Tim Willits, the lead designer on id Software's upcoming DOOM III (unless I miss my guess, this was conducted waaay back at QuakeCon). They discuss his inspirations in pursuing a career in game design, working with John Carmack, how DOOM III is coming along, challenges the game has presented, the most impressive feature of their new engine, multiplayer support, mod support, supporting lower-end hardware, weapons, per-poly collision detection, and more.

I think the problem is your perception of "little extra effort" here. Doom 3 is going to be VERY plot- (and therefore script-) driven, and as such, co-op would be extremely difficult to do. Every in-game event and sequence of events would be exponentially more complex as it would have to keep track of both players, and that raises all sorts of problems. I agree that it's a real pity. I'd like to see id put in a short co-op campaign, similar to the way NOLF2 has done. That would placate the co-op people (not that I think it's an unreasonable request) and wouldn't break the single-player dynamic or take an age to finish.
