I'm sure they have. Right before they see something off in the distance and go wandering off to find it. It's an open world game, keeping story direction is almost impossible if you make the world fun to explore. I got lost in the world of New Vegas too and for the record it wasn't that well written, just the sidequests and companion stuff was awesome.
It's really hard to keep that impact and interest from the player when there are 100 different things to do at any given time and you can't predict what the player will want.
