Can you give some example of the outcome then, what did the choice do? I'm seeing hundreds of reviews repeating that there are few instances of actual consequences and impactless side-questing.
"You go to the location, you kill everything that moves, grab the thing you need (if even) and that's it." Is that not true, or all these negative reviews playing a different game?

Side quests generally lack any sort of gray area and rarely have any lasting impact on the game. Main quests and faction quests are usually pretty straightforward, leaving players to wonder how many fetch quests or slaughter missions there will be before the next bit of story. There are tons of side quests to fill the void, but most are just combat-focused smash fests instead of complex dialogue-heavy ones.
- Regarding Armor -
I still see it as a problem. Its now something far more common and not something rare to fix up even if that beginning part is sort of a tutorial. Spoiler ->
The quest in particular forces you you into a situation where you have to interact with the armor in order to further interact with Preston Gravy, which is part of the main story. You can't interact with the Raiders and are forced to kill them, along with the Deathclaw.- With companions -
If there was some sort of Hardcore thing, there is no save scumming. Save scumming is a time honored tradition is most other RPG's anyways. So if Bethesda can't figure out of a way to make it less frustrating, then they should work on making that aspect better in their engine (tactical control/tactical view, etc.) rather than taking a lazy approach. I can see why they did it though (lots of people reporting companions still get stuck on the environment in this engine)-> that RPG review up above covers the lackluster companions.
