100% scan, completion, every mission done, Zaed not loyal (i choose the paragon choice here) and i admit that i sometimes skipped dialog but i always read every dialog line before skipping it.

Thats after fooling around 2 hours to solve the remaining things, and simply visit everything again. Also i spend a lot of time to talk to all my crew even after the main mission ended to find out all the dialog, so that added another 2 hours (you gotta fly around and go on-planet, of planet a while for it to "reset" )

I don't really know where else to make screenshots to show completion though, quest log is everything black and it doesn't fit on-screen and is in german so would be no help to you ,p

Edit: And yes, everyone apart from Zaed is loyal (and 0 people died on the final mission) which cost me 2 replays only (first time i lost Tali which was un-acceptable ,p)
