Well, if you can't be in an area with 500+ other players, and everywhere you go is an instance, it's not very massive. And you certainly shouldn't charge 15 bucks a month for it.
Guild Wars was similar, where you could see some other players in the towns but the rest of the game was instanced. It was more like Diablo, and no one called that an MMORPG. Nor did they charge a monthly fee.
If WoW was nothing but a few cities and instanced dungeons that you accessed by clicking on a map, it would have failed. (of course, for a lot of end game PvP players, that's pretty much all they do anyway.)
