Yes, in an ideal world the actor should take responsibility but that is not how it works on film sets. Most actors are overgrown children. It is 100% the responsibility of the armourer to protect the actors even from themselves. On rust the production company was negligent in hiring an armourer who was incompetent and also needed more trained assistants.
The state of New Mexico wanted the film to be shoot there so badly they allowed this to happen. The line producer and production manager are the real culprits (more people will die on low budget tv shows and movies until people like them are convicted).
