The average "poor" person in the US still has amenities that the middle class don't always have in an unindustrialized country -- everything from luxury items like televisions and cars to "basic necessities" like consistantly clean drinking water, consistent power and sewage, and other "little" things like that. And, as you mentioned, the average person in poverty in an industrialized country doesn't have to worry about plagues.
