To play Devil's Advocate (boy, I love that game)...
On one hand, there's so much red tape that to get even the smallest thing done takes way too much time and resources. Imagine the story of "Generator in US base in Iraq detonates killing a dozen soldiers, because Pentagon took weeks to send them replacement washers." An exaggeration, but you get the idea.
On the other hand, when they try to automate to get stuff done immediately, stuff like this happens.
I'm not saying it's impossible for there to be a balance between speed and oversight, they just don't have much experience with having both at the same time.
In either case, those sisters deserve a lot of jail time.

The Pentagon got scammed because of automated bill payment. Nobody was even looking at those invoices. That's exactly why I refuse to do automated bill payment at home... Hopefully by looking at the bills every month I have an extra layer of protection against fraud. People taking stuff out of my bank account automatically just scares me.

Shit, and I thought my 9 bucks for 2 day Fedex shipping on Bioshock was expensive.
