Finished my TVHM playthrough, and didn't get a single fucking orange. Attention devs: When your game is all about loot, MAKE SURE GOOD LOOT ACTUALLY FUCKING DROPS. Creston Good loot does drop. It may not be orange, but it's still good stuff. The thing is, you can continue playing and always get good gear without the need to grind, which is much better than D3. D3 has an additional problem where you do get "oranges" but they suck.
