My interpretation of the trailer is that the demons are "angels of death". It doesn't look like they're attacking him. More like swarming around so they can grab people as he takes them out. So basically a visual representation of a metaphorical situation.

I'd thought about that, but couldn't really remember if Max really saw demons or just warped people. From the trailer it looks like he's actually fighting them.
It also seems a bit weird that the director was going apeshit over an R rating vs a PG-13 rating. You're making a movie based off an R rated game. What exactly were you expecting?

The trailers have me a bit confused. Are those DEMONS flying at him, in the skyscraper bit? What the fuck are demons doing in Max freaking Payne? It's been ages since I played Max Panye but I remember that there was a drug mentioned in the game, named Valkyrie I think. I'm guessing that the creatures supposed to be halucinations from using it.
